Fill out this form completely and click SUBMIT before bringing your sample(s) to BI 208 for DNA sequencing.
After you click SUBMIT the summary/"Thank you" page will come up. You may want to print a copy of it for your records.
If you have more than 10 samples use your browser's Back button after the summary/"Thank you" page comes up. Then change the sample names and/or primer. That way you won't need to fill in the rest of the fields for each submission.
